This is a form required by Government under the Aged Care Volunteer Visitors Scheme guidelines.
This form is necessary enable you to be covered by the Golden Retriever Club of South Australia Inc’s insurance policy during your visits.
If you have been a citizen or permanent resident of another country other than Australia since turning 16 years of age, you will need to make a statutory declaration in addition to a police check.
